29 CFR 1926.501(b)(15): "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. a. On or about April 28, 2015 - at the above addressed jobsite, an employee performing roofing operations, climbed up on top of a steel beam and was exposed to a fall of approximately six (6) feet seven (7) inches to the lower roof, thereby exposing the employee to a fall hazard.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain shall be rigged and supported in such a way that its lowest point (including sag) is no less than 34 inches (.9 m) from the walking/working surface and its highest point is no more than 39 inches (1.0 m) from the walking/working surface; a. On or about April 28, 2015 - at the above addressed jobsite, the rope was rigged in such a way that its lowest point (sag) was at approximately thirty two (32) inches from the walking/working surface, thereby exposing the employees to falls of approximately twenty seven (27) feet to the ground.
