29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) West side of building, on or about 4-24-15: One employee exited a scaffold platform by jumping to the ground, exposing the employees to a fall hazard of 7 feet. b) West side of building, on or about 4-24-15: One employee accessed a scaffold platform while scaling over a ladder jack bracket, exposing the employees to a fall hazard of 7 feet. c) West side of building, on or about 4-24-15: One employee accessed a roof via a scaffold platform by jumping up 32 inches to the roof surface, exposing the employees to a fall hazard of 9 feet 8 inches. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
