Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) PAR Roofing, 3517 7th Ave. Charleston, WV: On or about 4/28/15, employees performing roofing activities on a low-slope roof were not protected from falling 17' to the ground. ABATEMENT CERTIFICATION IS REQUIRED.
