Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) Brock Industrial Services, 3517 7th Ave., Charleston: On or about 4/28/15, employees installing insulation on a low slope roof were not protected from falling 17' to the ground. ABATEMENT CERTIFICATION IS REQUIRED.
