29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Excavation, new sewer line construction located at 2691 Windy Hill Road, Marietta, GA 30067: An employee performing sewer line work inside an excavation which measured approximately 10-feet, 4-inches wide by 42-feet long by 8-feet deep was not provided with a safe means of egress, i.e. ladder and or ramp.

29 CFR 1926.652(a)(1): Each employee was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) Excavation, new sewer line construction located at 2691 Windy Hill Road SE, Marietta, GA 30067: An employee was allowed to work in a excavation approximately 8-feet deep, in a class "B" soil that was protected improperly with inadequate sloping instead of with the required sloping or shoring system. Construction Management & Engineering Services (CMES, Inc.) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 652(a)(1), which was contained in OSHA inspection number 314636176, citation number 1, item number 1 and was affirmed as a final order on 11/02/2010, with respect to a workplace located at Corner of Lake Drive NE and Jamerson Road NE, Marietta, GA 30066.
