29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) On or about 04/23/2015 - PACMAC Vertical Bagging Machine, Model #9500, Serial #9500B3525, a bagger operator designated in the lockout / tagout control procedure as an "authorized" employee had only been trained as an affected employee and was injured when he attempted to clear a film jam without properly locking out the equipment.
