29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: 7777 Westbank Expressway (Bayou Segnette State Park), Westwego, LA - On or about April 21, 2015, on the front porch overhang area, employees were working from a mobile scaffold that was not equipped with guardrails. The employee was exposed to a fall hazard of 10 1/2 feet to the ground.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: 7777 Westbank Expressway (Bayou Segnette State Park), Westwego, LA: On or about April 21, 2015, at the front of the job site, the employer did not ensure that adequate strain relief was provided on the power cord to a Dewalt chop saw used to cut metal frames.
