29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Employees engaged in roofing activities were exposed to injury from a fall hazard in that they had not received training. Roofing employees had not been trained to recognize, evaluate, and control fall hazards or in the use and limitations of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employees engaged in roofing activities were exposed to injury from a fall hazard while accessing and exiting the roof. The ladder that provided access to the roof on the side of the house did not extend 3 feet above the eave.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous. At the job site located at 5410 Roe Ave, Roe Land Park, KS, employees were exposed to an electrical shock hazard in that the ground prong was missing on the extension cord which was powering a circular saw connected to an extension cord was missing a grounding prong.
