29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about April 24, 2015, the employer failed to assure that approval was obtained by the manufacturer before adding a in house fabricated working platform to a Toyota forklift (model number 7FGCV35, Serial number 63332). The addition was used in the warehouse to lift maintenance workers.
