29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).    The employer does not guard the point of operation of machines whose operation exposes employees to injury.     a. This violation occurred on or about April 29, 2015, and at times prior thereto, in the manufacturing shop where employees were exposed to crushed-by hazards when operating an Atlantic (brand) press brake, serial number 57742, without guarding the point of operation guard.        b. This violation occurred on or about April 29, 2015 and at times prior thereto, in the manufacturing shop where employees were exposed to crushed-by hazards when operating a Durmazlan HAP 30160 press brake, serial number 7051024076, without guarding the point of operation.      Texas Metal Tech, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1037301, citation number 1, item number 2, and was affirmed as a final order on March 31, 2015, with respect to a workplace located at 7007 Belgold, Suite D, Houston, TX 77066.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the point of operation of machines whose operation exposes employees to injury is guarded.
