29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On April 29, 2015, on the main access road into Hickam AFB, employees working in a 5-foot, 2-inch deep section of a trench installing a 10-inch diameter water line, were not protected from cave-ins.
