29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Employees working on a pallet flow racking system were exposed to fall hazards of up to approximately 18 feet and the employer has not ensured that the anchorage for the personal fall arrest system (PFAS) was adequate, causing the PFAS to be ineffective.
