29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:    a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure that energy control procedures identified specific steps to follow to control hazardous energy of the Line 10 cutter.  Procedures lacked specific steps for equipment shutdown, isolation, placement of lockout devices and step to verify energy control measures.     b. Hearthside Food Solutions, LLC - McComb, Ohio: On or about May 20, 2015, the employer did not ensure that energy control procedures identified specific steps to follow to control hazardous energy of the Line 11 cutter.  Procedures lacked specific steps for equipment shutdown, isolation, placement of lockout devices and step to verify energy control measures.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure that employees were protected from hazardous energy sources while removing jammed/excess dough from the Line #10 rotary cutter.  The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating parts associated with the cutter to remove excess dough.  b. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 30, 2015, the employer did not ensure that employees were protected from hazardous energy sources while setting up the Line #10 cutter.  The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating part hazards to change the rotary cutter.  c. Hearthside Food Solutions, LLC - McComb, Ohio: On or about May 20, 2015, the employer did not ensure that employees were protected from hazardous energy sources while setting up the Line #11 cutter.  The machine was not shut down and energy isolating devices were not used when employees were exposed to rotating part hazards to change the rotary cutter. 29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure employees who service equipment such as, but not limited to, the rotary cutter at Lines 10 and 11, were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees that set-up the cutter and/or unjammed material from the cutter were not trained to isolate all energy sources during those tasks.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Hearthside Food Solutions, LLC - McComb, Ohio: On or about April 21, 2015, the employer did not ensure that employees were protected from rotating part hazards located at the Line #10 rotary cutter.  The cutter was not adequately guarded in that employees could bypass and/or reach through the guard while the machine operated.     Hearthside Food Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 313786212, citation number 1, item number 1and was affirmed as a final order on January 21, 2011, with respect to a workplace located at 312 Rader Rd. in McComb, Ohio 45858.
