29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolated:    At this establishment, at Gulf Wind Site, employees (maintenance/wind technicians) were exposed to electrical, mechanical, stored, hydraulic, and kinetic energies, when performing maintenance on wind towers equipment and machineries. The employer did not train his employees in using the company's specific procedures for lock-out/tag-out for each equipment or machinery and in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolated. Maintenance crew employees had not implemented and used the company's specific procedures for lock-out/tag-out for each equipment or machinery such as, but not limited to the following:   a) Wind Tower Bottom Electrical Breakers Control Panel (Breakers - F2, F5, Q9 and Local Switch);  b) Wind Turbine G.O. Hydraulic Station Rotor (Pressure Shut Off/Isolation - Ball Valve V-101 and the blades);  c) Wind Tower Gear Box (high speed pin);  d) Converter Module Cabinet;  e) Mitsubishi Wind Turbine Generator - MWT62/1.0 (low speed locking pin) and the Capacitors;  f) Yaw System;  g) Wind Tower Top Electrical Breakers Control Panel.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   At this establishment, at Gulf Wind Site, and times prior thereto, the employer failed to develop, implement, and /or maintain at the work site a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h). During the inspection and through employees interview, the CSHO observed that the employer did not have and maintain all SDS available to the employees; and the employer was not providing to all employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, when working with hazardous chemicals, such as but not limited to:  1. Resin Solution (mixture of isomers - Styrene, Xylene, Methacrylic Acid);  2. Synthetic wind turbine gear oil (MOBILGEAR SHC XMP 320);  3. Hydraulic oil (MOBIL UNIVIS N32);  4. Universal Metallic Spray/Aerosol paint (Rust-oleum);  5. Industrial cleaner and degreaser (Simple Green).

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:(a)    At this establishment, at Gulf Wind Site, and times prior thereto, the employer did not provide an educational program (training) for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting. During the inspection and through employees interview, the CSHO observed that the employer policy (Employee Safety and Health Handbook) was stand for fighting incipient stage fire fighting by using the fire extinguishers, but the employees never received any training on how to use them.
