29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    Throughout facility - On or about April 18, 2015, the lockout tag out procedures for equipment such as, but not limited to the Formax F6 failed to clearly identify all of the specific energy sources and steps to be followed by employees to de-energize the equipment and control the hazardous energy.  Employees were thereby exposed to the unexpected re-energization of machinery.

29 CFR 1910.147(c)(7)(i):     The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. 29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tag out device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  Throughout facility - On or about April 18, 2015, personal lockout or tag out devices were not attached to a group lockout device when two employees were troubleshooting the Formax F6 and other operating equipment, thereby exposing employees to the hazards of the unexpected re-energization of equipment resulting in an amputation injury.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a.  Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number 039, was not adequately guarded, thereby exposing employees to amputations.      b.  Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number 245, was not adequately guarded, thereby exposing employees to amputations.    c.  Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number D6FN54, was not adequately guarded, thereby exposing employees to amputations.    d.  Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number D7GT05, was not adequately guarded, thereby exposing employees to amputations.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.     a.  Production Area - On or about April 29, 2015, the employer failed to guard the rotating parts on the Lance tumblers, model number LT60, serial number 81993, thereby exposing employees to the hazards associated with rotating parts.     b.  Production Area - On or about April 29, 2015, the employer failed to guard the rotating parts on the Lance tumblers, model number LT60, serial number 86196, thereby exposing employees to the hazards associated with rotating parts.       c.  Production Area - On or about April 29, 2015, the employer failed to guard the rotating parts on the Lance tumblers, model number LT60, serial number not legible, thereby exposing employees to the hazards associated with rotating parts.    Koch Foods, Inc. dba Aspen Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 957280, citation number 1, item number 1 and was affirmed as a final order on March 19, 2014, with respect to a workplace located at 1115 West Fulton Street, Chicago, IL 60607.
