The Electric Materials Company, 50 South Washington Street, North East, PA 16428: Foundry Area - On or about April 29, 2015, and at times prior thereto, employees periodically dispensed a flammable liquid, Top Bond B, from a fifty-five (55) gallon drum into a container. The flammable liquid was dispensed from the drum through a quarter turn valve and small pipes.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a.) The Electric Materials Company, 50 South Washington Street, North East, PA 16428: DC Caster Area - On or about April 29, 2015, and at times prior thereto, employees were potentially exposed to burn hazards from molten metal while conducting kettle stirring operations. Aluminized jackets and aprons were not utilized by employees while conducting kettle stirring operations to prevent thermal burns to the torso, arms, and legs.

The Electric Materials Company, 50 South Washington Street, North East, PA 16428: Plating Area, Sulfuric Acid Dip Tank Area - On or about April 29, 2015, and at times prior thereto, employees worked with and around corrosive substances including, but not limited to, sulfuric acid and potassium hydroxide. The eye washes and safety showers in the areas were not inspected weekly to ensure proper operation. B.) The Electric Materials Company, 50 South Washington Street, North East, PA 16428: Foundry Area - On or about April 29, 2015, and at times prior thereto, employees worked with and around a corrosive liquid, Alpha Set 9040. Suitable facilities for the quick drenching and flushing of the eyes and body were not available in the area where the corrosive liquid was used.
