29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, or physical contact:    a) Residential jobsite: On April 28, 2015 and at times prior thereto an employee wore unserviceable footwear.  The condition of the footwear presented a slipping and tripping hazard to the employee while walking and working on a 6 in 12 pitched roof, carrying and stacking bundles of roofing materials.     Note) Certification of corrective action IS required for this item.
