29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) The Electric Materials Co. / 50 South Washington Street, North East PA: On or about April 29, 2015 a Powermatic bandsaw located in the shipping department and used for cutting paper, cardboard and wood for packing purposes did not have guarding to protect the unused portion of the saw blade during cutting operations.

An attempted repair was made with splicing and electrical tape.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) The Electric Material Co. / 50 South Washing Street, North East, PA: On or about April 29, 2015 in the plating area, an outlet with a knockout style receptacle box was not mounted or secured and was setting on a piece of equipment in the plating area. The knockout style receptacle box was being used as a permanent extension cord for the silver plating area.
