29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: At this residential construction site, and other times prior, throughout this work site under construction, plywood, protruding nails on boards, trash were left on the ground walking pathways exposing employees to impalement, trip and fall hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. At this commercial residential building site, the electrical wires on a flexible electrical cord being used to power equipment, showed strain relief and damage and could cause an electrical shock hazard to employees.
