29 CFR 1910.38(f)(1): Review of emergency action plan. An employer must review the emergency action plan with each employee covered by the plan when the plan is develop or the employee is assigned initially to a job: a. On or about April 30, 2015, in the facility, employees and temporary employees, were exposed to hazards including but not limited to, fire and ammonia when they had not been provided with adequate training on the employer's emergency action plan. Employees were not trained on the types of emergencies possible and the procedures used to account for employee(s) after an evacuation.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. a. On or about April 30, 2015, in the facility, employees and temporary employees, were exposed to hazards, including but not limited to ammonia, nitrogen, and chlorine dioxide. The employer failed to provide employees with effective information and training on hazardous chemicals in the facility. The Stampede Meat, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 315156612, citation number 1, item number 7(b) and was affirmed as a final order on June 28, 2011, with respect to a workplace located at 7351 South 78th Avenue, Bridgeview, IL 60455.
