29 CFR 1903.19(c)(1): Within ten calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section:  a.  The employer has failed to submit abatement certification within ten days of the abatement date, June 25, 2015, for a violation of 29 CFR 1926.100(a), issued as citation 1, item 1, and described as follows:      At the worksite where employees were working below employees on the roof, employees were not protected by protective helmets, thereby being exposed to struck-by hazards.  b. The employer has failed to submit abatement certification within ten days of the abatement date, June 25, 2015, for a violation of 29 CFR 1926.501(b)(1), issued as citation 1, item 2, and described as follows:     On the South side of the worksite where an employee was installing edging on the roof of the drive up window, the employee was not protected from falls, thereby being exposed to a 12 foot fall hazard.  c. The employer has failed to submit abatement certification within ten days of the abatement date, June 25, 2015, for a violation of 29 CFR 1926.1053(b)(4), issued as citation 1, item 3, and described as follows:     On the South side of the worksite where an employee was using a ladder to gain access to the drive thru roof area, the ladder was placed two feet from the side of the roof requiring the employee to step off the side and onto the roof, thereby exposing the employee to a 12 foot fall hazard.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a. At the worksite where employees were working below employees on the roof, employees were not protected by protective helmets, thereby being exposed to struck-by hazards.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On the south side of the worksite where an employee was installing edging on the roof of the drive up window, the employee was not protected from falls, thereby being exposed to a  12 foot fall hazard.

On the south side of the worksite where an employee was using a ladder to gain access to the drive thru roof area, the ladder was placed 2 feet from the side of the roof requiring the employee to step off the side and onto the roof, thereby exposing the employee to a 12 foot fall hazard.
