29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not ensure that personal protective equipment that protected the employee's body from hazards associated with methylene chloride and acetone was provided. While applying adhesives which contained methylene chloride and acetone, employees wore aprons which were not impervious to the chemicals. Employees were at risk of adverse health effects from exposure to methylene chloride and acetone including but not limited to skin irritation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not assess the workplace to determine what hazards chemicals presented in the workplace. Employees used adhesives which contained methylene chloride, acetone, and methylene bisphenol isocyanate to prepare printing screens. Employees were at risk of adverse health effects from exposure to hazardous chemicals including but not limited to skin irritation.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes: (a) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not select and require employees to use gloves that would protect employees from skin contact with methylene chloride and acetone. The employer provided Ansell Sol-vex nitrile gloves which are not recommended by the manufacturer for protection from methylene chloride and acetone exposure. Employees were exposed to health hazards from skin absorption of methylene chloride and acetone and at risk of adverse health effects including but not limited to skin irritation and lassitude. (b) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not select and require employees to use gloves that would protect employees from skin contact with methylene chloride and acetone. The employer provided Radnor nitrile disposable gloves. Nitrile has a short permeation time and is not recommended for methylene chloride and acetone. Employees were exposed to health hazards from skin absorption of methylene chloride and acetone and at risk of adverse health effects including but not limited to skin irritation and lassitude.

29 CFR 1910.1052(d)(3): Where the initial determination showed employee exposures at or above the action limit or above the short term exposure limit, the employer did not establish a periodic exposure monitoring program for methylene chloride in accordance with Table 1: (a) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not conduct periodic monitoring of the work place in accordance with Table 1. Analytical results revealed employee 8-hour time weighted average exposures to methylene chloride at 14 ppm and short term exposure at 290 ppm. The required semiannual, 8-hour time weighted average monitoring and the quarterly, STEL monitoring were not conducted.

29 CFR 1910.1052(l)(3)(i): The employer did not inform each affected employee of the requirements of 29 CFR 1910.1052 and the information available in its appendices and/or how to access or obtain a copy of it in the workplace: (a) Schilling Graphics, Inc. 275 Gelsanliter Road, Galion Ohio: On or about March 30, 2015, the employer did not inform the employees, who were using an adhesive which contained methylene chloride, of the requirements of the OSHA methylene chloride regulation (29 CFR 1910.1052) and the health and safety information contained in appendix A to the regulation. Employees were unaware of the hazards, signs, and symptoms associated with methylene chloride exposure.
