29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds: (a) Jobsite: On or a bout April 24, 2015, the employer did not provide fall protection for two employees who were erecting a tubular welded frame scaffold. The fall hazard to the two employees was about 16 feet to the ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite: On or about April 24, 2015, two employees were working (laying bricks) from a roof and they were not protected from falling. The roof had about a 24 inch high parapet. The fall hazard to the two employees was about 18 feet to the ground.
