29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a- 39 Fifth Avenue, Lavallette NJ: An employee was exposed to fall hazards, in excess of 18 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was missing end stops and a midrail/screen.  Condition observed on or about 04/28/15.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
