29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about April 28, 2015 at the Edwards Fiberglass Inc. facility in Sedalia, Missouri- employees were exposed to a struck by hazard from an Ace Dual Light Bench Grinder, in the Maintenance Shop, which had tongue guards that were improperly adjusted in excess of one-fourth inch. Abatement certification is not required for this item:

On or about April 28, 2015 at the Edwards Fiberglass Inc. facility in Sedalia, Missouri -employees in the maintenance shop were exposed to a fire hazard from gas cylinders which were stored in direct contact with oxygen cylinders.
