29 CFR 1926.451(b)(6): On scaffolds where scaffold planks were abutted to create a long platform, each abutted end did not rest on a separate support surface: On or about April 30, 2015, during the instillation of exterior vinyl siding on a new residential home, the employer did not ensure that the abutted ends had their own support surface. The sub-contractors employees were exposed to an approximate 13.6 foot fall.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. On April 30, 2015, during the installation of vinyl siding on a new residential home the employer did not ensure that the subcontractors platform did not extend less than 18 inches over its support while using a pump jack scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about April 30, 2015, during the instillation of exterior vinyl siding on a new residential home, the employer did not ensure that the sub-contractors employees were protected from a fall hazard of up to approximately 13.6 feet by utilizing fall protection systems/devices.
