A. Mark Blair DBA Blair Construction worksite located at 6694 St. Rt. 274, Celina, Ohio: On or about April 30, 2015, the employer did not ensure the 11ft x 4ft working platform was securely attached to the forks. The unsecured platform was at a height of approximately 9.00 feet while employees installed plywood sheathing.
