29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  (a) On or about May 14, 2015 in the chicken processing room, in the grinding room and at the entry door to the refrigerated areas, electrical cable had been ran at heights which created a tripping hazard or obstruction.  The employees were exposed to electrical hazards that could cause burns, shocks or death if the line is pulled from the equipment end, and trip hazards that could cause bruises and contusions.

29 CFR 1910.176(b): Storage was not secure. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:  (a)  On or about May 14, 2015 in the dry, refrigerated, and freezer storage areas, the boxes were found to be stacked haphazardly on racks.  An empty pallet had fallen from a higher level and was stopped by the storage below it, and a rack holding a pallet was not level and tipped forward due to the rack having been damaged.  USDA employees were exposed to being struck by overhead objects when inspecting these areas for vermin and proper storage.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about May 14, 2015 in the chicken room and the red meat room at the scales, the employer was using Kensington and Tripp Lite surge protectors in an industrial setting and in a wet or damp area.  This usage is not allowed under the list UL 1449.  Employees were exposed to possible electric shock and burns.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating:   (a)  On or about May 14, 2015 in the red meat processing room at the scale, the Kensington surge protector used on the computer/scales for red meat had the side broken out, exposing the interior electrical wiring.  Employees were exposed potential electrical shock and burns.    (b)  On or about May 14, 2014 in the freeze tunnel room, the electrical junction receptacle on top of the freeze tunnel had corroded and wiring was exposed.  Employees were exposed to potential electrical shocks and burns.

The employer did not have a procedure to control the electrical, hydraulic, and chemical energy present in the machine.
