29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    Employees who were performing roofing work were not protected from falling more than 8 feet to the lower level.    Reynaldo Rufino-Octaviano was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 893453, citation number 1, item number 1 and was affirmed as a final order on April 4, 2013, with respect to a workplace located at 6211 Velasco Avenue, Dallas, TX 75214.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Employees performing roofing work used an extension ladder that had side rails that did not extend at least 3 feet above the upper landing surface.    Reynaldo Rufino-Octaviano was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 893453, citation number 1, item number 2 and was affirmed as a final order on April 4, 2013, with respect to a workplace located at 6211 Velasco Avenue, Dallas, TX 75214.
