29 CFR 1926.1053(b)(10): The top of a non-self-supporting ladder were not placed with the two rails supported equally. Location - 2 Technology Drive, West Lebanon, NH: On or about April 24, 2015, an employee was using an extension ladder that was only supported by one sheet metal stud while working at a height of approximately 10 feet. This created a situation where the ladder was unstable and created a fall hazard for the employee.

29 CFR 1904.39(a): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident: A work-related incident that resulted in a multi-day hospitalization of the injured employee beginning on April 24, 2015, was not reported to OSHA until April 30, 2015 which was beyond the 24-hour requirement for notification.
