29 CFR 1926.102(a)(3): The vision of employees requiring use of corrective lenses in spectacles was not protected by goggles or spectacles of one of the types listed in paragraphs (i) through (iii) of this section: (a) At the St. Frances Xavier Cabrini Catholic Church construction site, an employee was observed drilling holes in steel beams while wearing prescription eyewear that was not American National Standards Institute (ANSI-87) rated safety eyewear and exposed to impact hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At the St. Frances Xavier Cabrini Catholic Church construction site, employees were using a 120V/14 AMP, High Speed Hougen-HDM 508 Drill powered by a 100-foot 12/3 AWG extension cord with ground pin missing.
