29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. a) East Hartford Facility, Aisle MA001 - On or about 4-30-15 - Employee operating a Crown FC4500 fork lift could not see employee walking into main aisle way from cross aisle that had a ten and a half foot high by four foot wide stack of palletized soda bottles obstructing his view.

29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed. a) East Hartford Plant, Warehouse Section - On or about 4-30-15 - Employee was struck by a moving fork lift when he walked directly into the main aisle way of the warehouse. The operator did not slow down or sound horn at cross aisle that was the employees main aisle way entrance that had ten and a half foot high by four foot wide pallets of soda stacked at the end of the aisle way.
