29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle:     Pro Printing NY Printing shop    a)  The employer failed to implement appropriate energy control procedures to protect employees using an UV-Coating  (JLG model SGZ-UV740A) machine that was undergoing repairs, where the guard of the pulley and belt have been removed; on or about 02-19-2015.    b)  The employer failed to implement appropriate energy control procedures to protect employees using an UV-Coating  (JLG model SGZ-UV740A) machine that was undergoing repairs, where the employees have been instructed to manually spin an unguarded pulley and belt to "lower" the feeding table; on or about 02-19-2015.
