29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On or about 4 May 2015, and at times prior thereto, Portable Storage Unit #1 had two empty Oxygen cylinders stored within 20 feet of an empty Acetylene cylinder, three full or partially full Propane canisters, a partially full 55-gallon drum of Synthetic Gear Lubricant, and a fall 55-gallon drum of Synthetic Gear Lubricant, exposing the employees working nearby to a struck by, fire and explosion hazard.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling. On or about 4 May 2015, and at times prior thereto, Re-locatable Power Taps (RPTs) were being used in a manner that was not in accordance with the manufacturer's instructions, exposing employees working nearby to a fire hazard. Instance A) Within the administrative building, behind the copier, an RPT was plugged into a Tripp-Lite Uninterruptible Power Supply, which was plugged into a four-plug outlet. The RPT had all six plugs being utilized at the time of the inspection, with one plug having an orange extension cord plugged in. The items plugged into the RPT were mainly for computers and computer accessories. Instance B) Within the administrative building, behind the tablet charging station, a total of four RPTs were plugged into a Panamax Uninterruptible Power Supply, which was plugged into a two-plug wall outlet. The RPT's were providing power to 20 tablet chargers at the time of the inspection. Instance C) Within Portable Storage Unit #3, on the west wall, an RPT was plugged into a three-plug extension cord tree, which was plugged into a two-plug outlet. The RPT was providing power to hand-held tools and chargers at the time of the inspection.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that could adversely affect safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. On or about 4 May 2015, and at times prior thereto, the employer allowed damaged electrical equipment to be utilized within the workplace that adversely affected the safe operation or mechanical strength of the unit, exposing employees to an electrical hazard of approximately 120 volts. The unit was a six outlet Belkin Surge Protector, Model F5C047 that had a cracked housing with some degree of separation.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. On or about 4 May 2015, and at times prior thereto, within Portable Storage Units #1-3, the overhead fluorescent lights were missing a protective screen or cover allowing the bulbs to be contacted at a height of under 8 feet, exposing the employees working under the lights to the hazard of being struck by falling shards of glass to the skin and eyes.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose. On or about 4 May 2015, and at times prior thereto, a junction box within Portable Storage Unit #3 was missing the cover allowing the energized wires inside to be contacted, exposing the employees working in the area to an electrical hazard of approximately 120 volts.
