29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Along the north side of the construction building, on or about May 04, 2015, employees were exposed to fall hazards of approximately 24 feet in height while installing metal form from a Snorkel aerial lift model TB-60DZ without wearing a body harness with lanyard secured to the basket.
