Employees did not know the trigger height for fall protection in construction on or about April 16, 2015.    New Millenium was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.503(a)(1) which was contained in OSHA Inspection Number 983589, Citation #1, Item #1b, issued on October 9, 2014 with respect to work place located at 1012 Hope Street, Philadelphia, PA 19123 and became final order on November 18, 2014.
