29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: a. Throughout Facilities - On or about April 30, 2015, the employer did not provide training to all affected, and all other employees to include all of the elements in (a) through (c), thereby exposing employees to unexpected re-energization of equipment causing severe crushing and amputation type injuries.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. Throughout Facility - On or about April 30, 2015, the employer did not guard the laminator, L70, serial 35931-24, model 192D, thereby exposing employees to amputations and the point of operation. b. Throughout Facility - On or about April 30, 2015, the employer did not guard the laminator, L71, serial 35932-35, model 192D, thereby exposing employees to amputations and the point of operation. c. Throughout Facility - On or about April 30, 2015, the employer did not guard the laminator, L72, serial 35933-34, model 192D, thereby exposing employees to amputations and the point of operation. d. Throughout Facility - On or about April 30, 2015, the employer did not guard the laminator, L74, serial 36241-01, model 192D, thereby exposing employees to amputations and the point of operation. e. Throughout Facility - On or about April 30, 2015, the employer did not guard the laminator, L76, serial 35809-01, model 192, thereby exposing employees to amputations and the point of operation.
