29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a. 14 Neuville Dr., Somerset (Franklin Twp), NJ     Employees engaged in foundation drain installation activities in a trench, approximately 8.5 feet deep were exposed to head injuries from soil and stones adjacent to the North edge of the trench falling into the trench as the employer had not provided hardhats to be worn. 29 CFR 1926.651(c)(2):   A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a) 14 Neuville Dr, Somerset (Franklin Twp), NJ    Employee(s) where not provided a safe means of egress while working inside a 50 foot section of a 75 foot long trench in depths of up to 8.5 feet, where the single means of egress was the end of the trench that was approximately 50 feet of lateral travel from where a cave-in occurred and where workers had been working.                                                                       Violation occurred on or about May 4, 2015
