29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Two employees were observed installing flashing on the south side of a commercial structure while not utilizing an adequate fall protection systems. Potential fall height was measured to be approximately 9'-9" with a roof pitch of 6/12.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  Employees were observed performing flashing work from a steep slope commercial roof without utilizing any personal fall arrest system exposed to a potential fall height in excess of 9' without the employer verifying and maintaining a written training certification record indicating the employees had received adequate training.
