29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. The frame was used for access.  On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; an employee was exposed to fall hazards of 11 feet, 6 inches to the lower level where observed taking measurements from the walk boards of the masons scaffold.

29 CFR 1926.451(e)(9)(iii): Employees erecting and dismantling tubular welded frame scaffolds (end) frames, were not provided with horizontal members that were parallel, level and 22 inches or less apart vertically to be used as climbing devices for access.  On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; a worker was exposed to fall hazards of up to 6 feet to the lower level where observed scaling the frame of the fabricated frame scaffold during the erection process, that was not designed for climbing.

On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; an employee erecting a fabricated frame scaffold was observed  exposed to fall hazards of 6 feet to the lower level while scaling the frame for access and egress. The employee was not a competent person and was not trained or knowledgeable of the requirements to safely erect a scaffold.
