29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Roof, residential home located at 2665 Pine Bloom Drive, Roswell, GA 30076: Employees operating Hitachi (Model NV45AB2) Coil Roofing Nail Guns at 105 PSI were not provided with protective eyewear. Employees operating these tools failed to wear safety glasses and goggles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Upper roof, residential construction located at 2665 Pine Bloom Drive, Roswell, GA 30076: Employees were exposed to falls of approximately 20-feet while engaged in roofing activities on a 7/12 pitched roof with no fall protection system in place.   (b) Lower roof, residential construction located at 2665 Pine Bloom Drive, Roswell, GA 30076: Employees were exposed to falls of approximately 9-feet, 1-inch while engaged in roofing activities on a 4/12 pitched roof with no fall protection system in place.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                   (a) Front side, residential home located at 2665 Pine Bloom Drive, Roswell, GA 30076: Employer allowed employees to use the TranzSporter (Model LH200) Platform Hoist as a ladder to access the roof. Manufacturer's general safety instructions state that this hoist should never be used as a ladder.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Front side, residential home located at 2665 Pine Bloom Drive, Roswell, GA 30076: The ladder provided was not secured to the structure and did not extend at least 3 feet above the landing surface of the roof to allow for safe access on and off the roof. Employees accessing this ladder were exposed to falls of approximately 9 feet, 1-inch to the lower level.
