29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. Mail processing area; on or about 05 May 2015 and times prior thereto; two instances were noted where energized electrical wall outlets had damaged faceplates: a. Inbound/outbound area; an energized electrical wall outlet had a damaged faceplate. b. Adjacent to the electrical room; and energized electrical wall outlet was missing the protective cover. This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Garage bay adjacent to custodial equipment area; on or about 05 May 2015 and times prior thereto; the Pure Flow 1000, emergency eyes wash station was blocked. A Hyrdo-Retriever floor scrubber and a roll-away cart were stored in front impeding immediate access. This condition exposed employees to eye trauma hazards.
