29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not constructed to prevent electrical shock: (a) On April 28, 2015 the inspection revealed that employees were potentially exposed to serious injury from electric shock because the rubber gasket on the pendant control box was damaged and was no longer water tight. Located on the fish salvage crane at the A Canal Headworks in Klamath Falls, Oregon.

29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: (a) On, about and at times prior to April 28, 2015 the inspection revealed that the employer did not measure the hoist chain on the yellow crane for stretch monthly or before each use. The crane was located at the A canal headworks in Klamath Falls, Oregon.
