General-duty citation text
29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails. Highway 97: On or about April 28, 2015 and times prior thereto, the ladder coming out of the space did not extend above the landing or did it have any grab bars to assist employees. This exposed employees to an eight foot fall into the space.
