29 CFR 1926.405(b)(2): In an energized installation, each outlet box did not have a cover, faceplate, or fixture canopy. Building 219: On or about April 29, 2015 and times prior thereto outlet boxes that were being used did not have covers installed. This exposed employees to shock/burn hazard from incidental contact with the conductor.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. Shower Area: On or about April 29, 2015 and times prior thereto, the general contractor did not insure that all outlets and GFCI's were in working condition. This exposed employees to potential shock hazard from using an ungrounded circuit.
