29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) At the facility, on or about May 5, 2015 and at times prior to, employees were exposed to a hazard of being struck by powered industrial trucks (PITs). 1) The employer did not remove from service a Toyota LTM forklift #16562 which did not have functioning left head light and rear tail light for use during operations.
