29 CFR 1926.34(a): "General." In every building or structure exits shall be so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it is occupied. No lock or fastening to prevent free escape from the inside of any building shall be installed except in mental, penal, or corrective institutions where supervisory personnel is continually on duty and effective provisions are made to remove occupants in case of fire or other emergency: a) On or about March 18, 2015 - at the jobsite located at 7109 S. Bennett Avenue, Chicago, IL. - employees working in a rehab building were using stairwells that were not properly maintained, thereby exposing employees to tripping and fall hazards. b) On or about March 18, 2015 - at the jobsite located at 7109 S. Bennett Avenue, Chicago, IL. - employees were working in a rehab building that had poor housekeeping, thereby exposing employees to safety and health hazards.

Construction areas, ramps, runways, corridors, offices, shops, and storage areas shall be lighted to not less than the minimum illumination intensities listed in Table D-3 while any work is in progress: a) On or about March 18, 2015 - at the jobsite located at 7109 S. Bennett Avenue, Chicago, IL. - employees were working in a rehab building using corridors, stairwells and working in various units that were not provided temporary lighting. The employee work zones were not properly illuminated, thereby exposing employees to tripping and fall hazards.

Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) On or about March 18, 2015 - at the jobsite located at 7109 S. Bennett Avenue, Chicago, IL. - employees working in a rehab building in second and third floor units near unprotected wall openings/windows were exposed to fall hazards of approximately twenty (20) to thirty (30) feet.
