29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)    On or about April 28, 2015:  Employees performing fireproofing operations in the kitchen area of Peachcrest Elementary School construction site located at 1530 Joy Lane in Decatur, Georgia are exposed to inhalation hazards such as, but not limited to Quartz, Aluminum sulphate, and Portland cement.  The employer requires the use of a respirator when mixing and spraying, but had not established and implemented a written respiratory protection program.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103)    On or about April 28, 2015:   The employee at the Peachcrest Elementary School construction site in the kitchen area working as the fireproofing helper and wearing the N95 filtering face piece respirator voluntarily, was exposed to inhalation and skin contact hazards.  The employer did not provide the employee with information in Appendix D of the respiratory protection standard.
