29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the openings:   Jobsite, rooftop: On, or about, 05/05/2015 employees were performing construction activities on a walking/working surface in and around two unguarded skylights and an unguarded open ventilation shaft and exposed to a fall greater than six feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   Jobsite: On, or about, 05/05/2015 employees were engaged in roofing work on a low-sloped roof and exposed to a fall of approximately 40 feet to the lower level whereby they were not protected by a guardrail system, net system, or personal fall protection system.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  Jobsite: On, or about, 05/05/2015 the extension ladders in use had a structural defects to the rungs and missing the ropes from the pulley systems and were not immediately marked in a manner identifying it as defective or tagged with DO NOT USE and immediately removed from service until repaired in the following instances:  (a) Rooftop, lower level access: The Werner aluminum extension ladder had damaged rungs and an incomplete rope pulley system.  (b) Rooftop, upper level access:  The Werner aluminum extension ladder had damaged rungs, missing a non-slip foot, and an incomplete rope pulley system.
