29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On the east side of the construction site, an employee engaging in masonry activities worked from a tubular welded frame scaffold without fall protection was exposed to fall hazards of approximately 16 feet.

29 CFR 1926.602(c)(1)(vi): Section 603(e) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed: On the east side of the construction site, the employees were exposed to struck by hazards while working with a forklift unattended and the forks elevated while installing masonry material on a commercial building.
